A duodenal villous adenoma associated with in situ carcinoma: A case report

چکیده
Villous adenomas of the duodenum behave in a manner similar to villous adenomas of the colon and rectum, having both a high rate of recurrence after local treatment and a high incidence of malignant transformation. Villous adenomas of the duodenum are the most common benign periampullary neoplasms. They are considered as premalignant lesions with a malignant transformation rate as high as 60%. Periampullary carcinomas arising from the villous adenomas of the duodenum are also the most frequent extracolonic cause of death in Familial Adenomatous Polyposis patients. Although the Whipple procedure remains the procedure of choice in treating adenomas containing invasive cancer, the selection of an appropriate procedure to treat benign villous adenomas of the duodenum, especially those with carcinoma in situ is controversial. Here we report a 63-year old woman who presented with epigastric pain secondary to a mass of the ampulla of Vater. An endoscopic biopsy revealed adenocarcinoma while pathological workup after curative pylorus preserving pancreaticoduodenectomy revealed villous adenoma of the duodenum associated with carcinoma in situ. BACKGROUND Villous duodenal adenomas are rare, tend to malignant alteration and recurrency if surgical procedures are not sufficiently radical. They are mostly localized in the second portion of the duodenum and close to a papilla and are prone to malignant alteration. CASE REPORT In this paper we presented a 79-year-old female patient already operated on twice elsewhere due to obstructive ja...
متن کاملLymphoepithelial – Like Carcinoma with Papillary Transitional Cell Carcinoma of the Urinary Bladder Associated with Carcinoma in situ Changes of the Urothelium; A Case Report and Review of Literature
Lymphoepithelial - like carcinoma, is rarely recognized in the urinary bladder and less commonly occurs with papillary transitional cell carcinoma i.e. mixed pattern. Also, less uncommon is the occurrence of carcinoma in situ changes in the adjacent urothelium of these tumors. Here, a case of lymphoepithelial – like carcinoma and papillary transitional cell carcinoma associated with carci...
متن کاملTreatment Options for Villous Adenoma of the Ampulla of Vater
INTRODUCTION Duodenal villous adenoma arising from the ampulla of Vater has a high risk of malignant development. Excluding associated malignant disease prior to resection of an adenoma of the ampulla is not always possible. Therefore, the surgical procedure of choice to treat this rare tumour is still controversial. OBJECTIVE To evaluate retrospectively results of treatment of villous adenom...
متن کاملA case of urachal villous adenoma with high grade dysplasia focally bordering on adenocarcinoma in situ
The urachus, or the median umbilical ligament, is a vestigial structure of the allantois that connects the urinary bladder and the umbilicus. Urachal carcinoma, originally described by Hue and Jacquin in 1963, is a rare non-urothelial carcinoma. The incidence is estimated to account for 0.01% of all malignancies, 0.5% of all bladder tumors, and compose 20e40% of bladder adenocarcinomas. Most ur...
متن کاملCASE REPORT Serrated adenoma of the duodenum
The duodenum is the most common site for extracolonic adenomas in patients with familial adenomatous polyposis (FAP). These adenomas are usually tubular, villous, or tubulovillous. This report describes a patient with FAP who had a duodenal adenoma with a different histology—it had the microscopic attributes of a serrated adenoma—tumours that are relatively common in the colorectal mucosa and a...
